Statement of Purpose

I wish to become a University of Arizona College of Pharmacy SWEHSC Volunteer.

I understand that I am required to submit all required UA and SWEHSC forms, complete an orientation, and attend occasional trainings. I will promptly update any of my application and personal information that changes during the period of my SWEHSC service, e.g. contact info or relating to background check/clearance or driving record. I will commit to being a reliable volunteer for SWEHSC's community outreach events, and will communicate promptly with SWEHSC staff if I must change my commitment unexpectedly before an activity.

Note: Typing your name in the signature block is sufficient for electronic submission.

The University of Arizona is an equal opportunity, affirmative action institution. The University of Arizona does not discriminate on the basis of race, color, religion, sex, national origin, age, disability, veteran status, or sexual orientation in its programs and activities.

Persons with a disability may request a reasonable accomodation, such as a sign language interpreter, by contacting the SWEHSC Office at 520-626-5594, or you may contact the Disability Resource Center at 520-621-3268 (V/TTY). Requests should be made as early as possible to allow time to arrange the accomodation.

Please complete the two additional forms, "Multi-Media Consent and Release Form for Individuals," and "Intermittent/Occasional Volunteer (IOV) Worksheet." The IOV Worksheet enables SWEHSC to provide liability insurance coverage for you as a UA volunteer and extend UA privileges, such as an email account and online access to campus library resources.
